Iker Altuna, 22, a process technician at the fictional solvent-recovery plant Quimica Vallmari S.L. in Castellon, prepares and oversees a contractor's welding job in a zone 2 area during a shutdown day.

Four rounds, each an action and a permit, surroundings, restart or reporting choice: isolating the pipe and the work permit; gas testing and preparing the surroundings; the gas detector alarm and what must happen before welding again; an ammonia leak from the refrigeration unit and the incident report.

Scored on safety margin, plant downtime per phase, avoidable damage (product, equipment, clean-up and stoppage) after what a good choice avoids, and a safety-barriers index. Three hard-fails cap the round at 40: hot work on a line not isolated or drained, silencing a gas alarm to finish hot work, and walking into an ammonia cloud without proper respiratory protection.
