About this simulation

A 90-minute, 4-round Express simulation on the outpatient revenue cycle of a US community practice: eligibility, copays and deductibles at the front desk, payer mix and contract renegotiation, claim denials by root cause, coding to documentation, uninsured and self-pay patients under the No Surprises Act, same-day access and no-shows, a Medi-Cal plan's value-based offer and patient balances under California's medical-debt law.

For health-administration, medical-office and billing programmes at Hispanic-Serving Institutions and healthcare-management electives. Bilingual EN/ES.

Ruiseñor Family Medical Group (a fictional seven-clinician family practice in Visalia, California): as Graciela, the new practice administrator, you decide how the front desk handles coverage and copays and what to do with the payer contracts, how to work denied claims and set visit levels, how to price care for the uninsured and run the schedule, and how to answer a Medi-Cal plan's offer and collect patient balances at year-end.

Scored on the operating result, the first-pass denial rate, days in accounts receivable, visits and patient trust.

Who it is for

An intermediate simulation for participants who already know the core concepts of the subject, designed for master's and MBA programmes.

How a session runs

  1. The instructor creates a session from the Eureka dashboard and invites the participants.
  2. Participants play 4 rounds. In each one they submit their decisions and the simulation calculates the results. The first round opens with: Monday 11 January 2027: the front desk and the contracts.
  3. The simulation can be run in English, Español.
  4. The instructor follows each participant's progress and results from the dashboard, and uses the class results for the debrief.

Decisions participants make

The decisions participants make during the simulation:

  • The front desk: before the visit
  • The payer contracts
  • The denied claims
  • The visit levels
  • Uninsured and self-pay patients
  • The schedule and the no-shows
  • Valle Central Health Plan's offer
  • Patient balances at year-end
