About this simulation

A 90-minute, 4-round basic Express simulation for Spanish vocational training (Formacion Profesional), module 1588 Sistemas de radiodiagnostico, radioterapia e imagen medica of the Tecnico Superior en Electromedicina Clinica (Real Decreto 838/2015).

The learner is Aitana Lizoain, a newly qualified electromedicine technician of a fictional authorised technical-assistance company, Servicios Electromedicos Arguedal, through 2027, in charge of the corrective maintenance of the X-ray rooms and the CT scanner of a fictional district hospital.

Four situations set the year's course: the room whose generator will not expose, and how the intervention is prepared under the hospital's radiation protection programme (the room out of clinical use, the generator isolated, test exposures only from behind the console's shield) and how the diagnosis starts (the error log read with the service manual); the rings in the CT, and how the fault is located (the manufacturer's fault tree, the phantom and the calibrations in order) and what part is fitted (only the faulty one, with the manufacturer's reference); the room that needs more exposure after its new tube, and how the cause is found (the generator calibrated after the tube change) and which quality control is done (the tests the intervention calls for, agreed with radiophysics); and how each item of equipment goes back to clinical use (verified and certified first, Real Decreto 1085/2009 article 11 g and Real Decreto 1976/1999 article 15.2) and how each intervention is recorded.

Three critical errors cap the round: test exposures with the door open and someone inside, the protocols' exposure raised to hide a fault, equipment back in clinical use before verification. KPIs: repairs conforming first time, imaging maintenance cost, hours out of service and spares changed without need. No clinical or dose decision is made by the learner. Spanish primary, English parity.

Fictional companies and people; real regulations and curriculum cited.

What participants learn

After the session, participants will be able to:

  • Prepare an imaging intervention under radiation protection: room withdrawn from clinical use in writing, generator isolated and discharged, dosimeter worn and test exposures only from behind the shield with the room empty
  • Analyze the error log and the service manual, reproducing the fault in service mode and checking the history, and write hypotheses down before replacing the board the code points to
  • Follow the fault tree with the phantom and calibrations in the manual's order to identify the faulty channel or module, and fit only the part with the manufacturer's reference for that model and serial number
  • Judge the quality-control tests a tube change calls for with radiophysics against the programme's tolerances after calibrating the generator, rejecting higher exposure protocols as a way to hide a fault
  • Plan the return to clinical use: equipment kept out until the post-repair test, quality control and restitution certificate are done, handed back in writing, with an intervention report the next technician can read

Who it is for

An introductory simulation that assumes no prior knowledge of the subject, designed for undergraduate courses. It also works in vocational and high-school programmes and in onboarding sessions.

How a session runs

  1. The instructor creates a session from the Eureka dashboard and invites the participants.
  2. Participants play 4 rounds. In each one they submit their decisions and the simulation calculates the results. The first round opens with: martes 9 de febrero, 7:40: la sala que no dispara.
  3. The simulation can be run in English, Español.
  4. The instructor follows each participant's progress and results from the dashboard, and uses the class results for the debrief.

Decisions participants make

The decisions participants make during the simulation:

  • Cómo se prepara la intervención
  • Cómo empieza el diagnóstico
  • Cómo se localiza la avería del TAC
  • Qué pieza se monta
  • Cómo se encuentra la causa tras el cambio de tubo
  • Qué control de calidad se hace
  • Cómo se devuelve el equipo al uso clínico
  • Cómo se cierra cada intervención
