A 90-minute, 4-round advanced Express simulation on hospital-wide bed management through a winter surge, for health-management programmes and for hospital management teams, admissions and patient-flow units and heads of clinical services.

Hospital Universitario de Peñalmoral, a fictional 610-bed public university hospital in Spain, December 2026 to March 2027: as its new deputy medical director for patient management you run the beds of services that do not want to share them, with a winter-plan budget of 565,000 euros.

Round by round you choose who owns the beds and how patients are discharged, what the hospital does when it is full and which surge capacity it adds, where medical outliers go and what happens to the patients medically fit but waiting for a social-care place, and how elective surgery is protected and what the services sign for next winter.

Each month is scored on the boarders waiting in the emergency department at 08:00, the average length of stay, the operations cancelled for lack of a bed, emergency readmissions, the services cooperation and spending against budget; trading beds in a messaging group with patients health data, staffing a winter ward on chained shifts without the minimum rest, or moving patients to a residence without their consent, caps the score.
