A four-round, advanced healthcare change-management simulation set inside Riverside Vale NHS Foundation Trust, a 620-bed Midlands district general hospital (GBP 410M budget, 6,400 staff) under a coroner's Regulation 28 'Prevention of Future Deaths' report.

A 34-year-old mother died of undetected sepsis; the Trust has 90 days to prove a credible plan to embed the nationally recommended 'Sepsis Six' bundle across its emergency department and two acute wards.

The evidence is settled — its own research-active sepsis team has published nationally — yet bundle compliance sits at 28% (a failed 2024 attempt peaked there and lapsed) and median time-to-antibiotics is 127 minutes against a 60-minute standard. The regulator expects a credible trajectory to ≥75% by day 90; the national reliability target is ≥90%.

Playing the Change Lead with only 0.6 WTE of protected champion time and a fixed GBP 180,000 budget, you (1) diagnose this as an ADOPTION gap, not an evidence or training gap, mapping barriers across Dopson's and the PARiHS lens of evidence, context and facilitation; (2) package the evidence and choose opinion-leaders, allocating champion time across the distant expert consultant, a trusted night-shift peer charge nurse, and managerial sponsors — the credible local peer often shifts practice more than the professor; (3) adapt to local context and sequence the rollout, choosing pilot-and-spread vs big-bang and the fidelity-vs-local-fit balance unit by unit; and (4) embed, measure and defend a day-90 trajectory to the Medical Director and a non-executive board member, naming leading and lagging indicators and an owner for each ward after the champion time ends.

The math rewards genuine, context-sensitive, peer-led adoption and punishes the five classic errors — re-proving the evidence or 'more training', the wrong messenger, one uniform mandate, total fidelity with no adaptation, and a launch event with no sustainment plan. Final KPIs track Bundle Compliance %, Adoption Quality (real vs paper), Sustainability, and Champion Time used against the 0.6 WTE cap.
